Contra Costa Health Services, Martinez, CA
The Health Officer of Contra Costa County requires that each and every licensed health care facility in Contra Costa County implement a program requiring its health care workers to receive an annual influenza vaccination or, if they decline, to wear a mask for the duration of the influenza season while working in patient care areas in that health care facility. This order applies to hospitals, ambulatory and community clinics, skilled nursing and other long term care facilities, and any other health care facility. For the purposes of this order, "health care workers" are persons, paid and unpaid, working in health care settings who have direct patient contact or who work in patient care areas. This order is ongoing and applies to each influenza season unless the order is rescinded. The influenza season is defined as November 1 to March 31 of the following year. In any given year, if influenza surveillance data demonstrate an unusually late peak and continued widespread influenza activity in the spring, the Health Officer may extend the period during which the masking program shall apply for that year.
In August 2014, the Health Officer of Contra Costa County issued a similar order requiring residential care facilities for the elderly (sometimes called "assisted living", "retirement home", or "board and care" facilities) to implement a program requiring its staff members to receive an annual influenza vaccination or, if they decline, to wear a mask for the duration of the influenza season while in direct contact with residents or while working in resident areas.

DaVita HealthCare Partners, Torrance, CA
All healthcare personnel (HCP), as well as clerical staff, are required to receive influenza vaccine. Employees who have not had the vaccine will be required to wear a surgical mask. All employees who have received the vaccine obtain a blue dot from the management team and place it on the lower left hand side of their badge. Failure to complete this requirement by a specified date is subject to corrective action, including and up to termination of employment with HCP.

Los Angeles County Department of Public Health, CA
The Los Angeles County Department of Public Health has issued an order mandating that healthcare personnel in acute care hospitals, long term care facilities, and intermediate care facilities in Los Angeles County be vaccinated against influenza, or wear a protective mask. The goal of the order is to lower the rates of transmission of influenza among healthcare personnel and the vulnerable populations that they serve. The influenza vaccination order applies to all individuals working in acute-care hospitals, long-term-care facilities, and intermediate-care facilities who have direct patient contact or work in patient areas during the influenza season (November 1 through March 31). Until it is rescinded, this Order applies to each influenza season, defined as November 1 of one year to March 31 of the following year.

Groups divided into two tiers. Tier one includes all staff, contract workers, students, and MDs with direct patient contact or contact with the patient care environment. Vaccines are mandatory for tier one group with masking in patient care areas mandatory if vaccine has not been received by 11/01/09. Tier two employees (those who do not work in a patient care environment) will be required to vaccinate or mask if and when vaccine supplies are determined to be adequate and flu levels rise.

In addition, medical declinations are to be signed by the employee's PCP and turned into employee health, and religious declinations will be reviewed by hospital pastoral staff. This policy applies to all of the Children's Hospital Network of Care locations throughout Colorado.

Colorado Department of Public Health & Environment, Denver, CO
The Colorado Board of Health passed a rule requiring Colorado's healthcare facilities to ensure annual influenza vaccination of their healthcare workers. The Colorado rule was a product of a yearlong collaboration between the Colorado Department of Public Health and Environment (CDPHE) and stakeholders across the state. As a direct result of the extensive collaborative approach taken, the final rule is unique among similar proposed and enacted state mandates in that it addresses a variety of healthcare facility needs and priorities relating to reporting, implementation, and enforcement. The rule applies to all licensed healthcare entities in Colorado. The Colorado rule was modeled to encourage facilities that are already appropriately implementing strategies to prevent influenza to continue to do so, to assist facilities that can improve, and to prompt facilities to adopt more effective policies to prevent influenza. The rule achieves this by allowing facilities that have already taken measures to ensure patient safety by promoting or mandating influenza vaccinations to be exempt from the rule if target vaccination rates (phased in to 90% in 2014) are reached. A facility can implement a policy that best fits its organization to achieve or exceed the target vaccination rates, including implementing any type of exemption or informed declination as long as it gets to the target vaccination rates described below. If a facility does not achieve the target vaccination rates, it is required to implement an influenza vaccination policy for healthcare worker that includes a medical exemption only and a requirement for workers that have not been immunized to wear a surgical mask during influenza season. As described above, the rule applies to all hospitals, ambulatory surgical centers, and long-term care facilities in the state. The rule also applies to other licensed healthcare entities in the state including community clinics, rehabilitation centers, community mental health centers, facilities for people with developmental disabilities, hospice care, assisted living residences, dialysis treatment clinics, birthing centers, home care agencies, maternity hospitals, psychiatric hospitals, convalescent centers and acute treatment units.

All healthcare workers both clinical and non-clinical are required to receive the seasonal influenza vaccination as a condition of employment. In addition, volunteers, students, vendors and contractors are required to receive the influenza vaccination. Exemption to immunization is only granted for valid medical and religious reasons as determined by a designated committee. Associates that are granted an exemption are required to wear a surgical mask while at work during the designated influenza season. Associates who refuse to receive the vaccination are counseled and provided an opportunity comply. Failure to comply will result into disciplinary action up to termination.

Traveling professionals working in our facility either were vaccinated or provided proof of vaccination from outside our facility to receive a "red dot" to their ID badge signifying vaccination. Medical, religious or personal choice exemptions were required to mask during active flu season when working with patients or within 6 feet of patients. Tuality Healthcare utilized multiple avenues to deliver the vaccine with roving carts, staff meetings, delivery of vaccine to multiple off sites for a key staff contact to vaccinate, and lunch-time station by cafeteria. Management promoted the campaign with our Chief Clinical Officer vaccination other administrators with a photo event.

Failure to comply to the policy will result in disciplinary action up to and including termination.

This requirement exists from October 1st through March 31st, unless the season needs to be extended based on severity.

In 2010, VM mandated H1N1 influenza vaccination for all, in addition to influenza vaccination.

North Bay Parry Sound District Health Unit, North Bay, Ontario Canada
All healthcare workers, as well as clerical and janitorial staff, must receive influenza vaccine or face dismissal. Exemption to immunization is only allowed for valid medical contraindications. After 2005 it is a condition of employment to have the influenza vaccine. Failure to do so without a National Advisory Committee on Immunization (NACI) approved medical contraindication/precaution will result in dismissal (there is no option for antiviral medication). Those hired prior to 2005 can opt for antivirals. If they refuse they are suspended without pay.

Peterborough County-City Health Unit, Ontario, Canada
Since 2002, mandatory immunization for influenza is included in the terms of employment, which a new employee signs. For new hires, exemption to immunization is only allowed for valid medical contraindications. In addition, we do allow for conscientious objection, but that is only for the staff who were hired before 2002. For staff hired before 2002, those who are not vaccinated must take antivirals to keep working in their assigned jobs, or they are reassigned to non-patient care duties and/or take holidays. For staff hired after 2002, they will be suspended without pay until they comply with the policy. Noncompliance could be grounds for dismissal.
