WHAT THE ADULT IMMUNIZATION RECORD CARD OFFERS
Providers. The card is a low-cost, easy-to-use teaching tool. Use it during
patient visits to discuss a patient's vaccination status with them. Then
give it to them and encourage them to use it to keep track of their next
vaccination due date.
Patients. Sized to fit in a wallet and printed on canary-yellow paper, the
card is a portable, easy-to-find personal immunization record. Printed on
smudge-proof, rip-proof, waterproof paper, it is intended for lifelong use.

NOTE: As currently configured, the Adult Immunization Record card does not
list two newly licensed vaccines--the tetanus-diphtheria-acellular pertussis
vaccine (Tdap) and the meningococcal conjugate vaccine (MCV4). Providers can
easily adapt the card, however, by writing Tdap in the space now designated
for Td vaccine and by writing MCV4 in the space now designated for "other"
vaccines.
