| Dr. Jakim is a board-certified family physician
who practices in Preston, Minnesota. |

| Dr. McCord is the Medical
Director of Pediatric Inpatient Services at The Children's Hospital at
Legacy Emanuel in Portland, Oregon. Dr. McCord was previously the
Medical Director of the Ambulatory Clinic and Medical Surgical Units at
the Saint Paul campus of Children's Hospitals and Clinics of Minnesota.
He graduated from Indiana University School of Medicine and is an
Adjunct Professor of Pediatrics at the University of Minnesota School of
Medicine. |

| Dr. Sheila Specker is both
family physician and psychiatrist and is Associate Professor in the
Departments of Family Practice and Psychiatry at the University of
Minnesota. Her specialties are addictive disorders and women's health
and she is medical director for the combined mental health/substance
abuse treatment program at the University. Her work combines education
of medical students, residents, and fellows; research in addictions; and
clinical practice. She has received federal grants and private funding
for projects. |

| Dr. Strodthoff is a
board-certified family physician who practices in Amery, Wisconsin. |

| Dr. Wexler, board-certified family physician, is
Executive Director and founder of the Immunization Action Coalition (IAC).
She is editor of IAC's periodicals for healthcare
professionals, Needle Tips
and Vaccinate
Adults, as well as IAC's electronic immunization news service, IAC
Express. Dr. Wexler's commitment to immunization
grew out of her medical practice delivering health care to Hmong refugees from Southeast
Asia as well as providing immunization services in non-traditional settings.
