WHAT THE ADULT IMMUNIZATION RECORD CARD OFFERS
Providers. The card is a low-cost, easy-to-use teaching tool. Use it during patient visits to discuss a patient's vaccination status with them. Then give it to them and encourage them to use it to keep track of their next vaccination due date.

Patients. Sized to fit in a wallet and printed on canary-yellow paper, the card is a portable, easy-to-find record of a patient's adult immunization history. It lets a patient know which vaccines are recommended for adults, which they've received, and when they received them. Printed on smudge-proof, rip-proof, waterproof paper, the card is intended for lifelong use.

NOTE: As currently configured, the Adult Immunization Record card does not list two newly licensed vaccines--the tetanus-diphtheria-acellular pertussis vaccine (Tdap) and the meningococcal conjugate vaccine (MCV4). Providers can easily adapt the card, however, by writing Tdap in the space now designated for Td vaccine and by writing MCV4 in the space now designated for "other" vaccines.

2. IMMUNIZATION TECHNIQUES VIDEO
Developed by the California Department of Health Services Immunization Branch in collaboration with a team of national experts, this 35-minute video teaches best practices for administering intramuscular (IM) and subcutaneous (SC) vaccines to infants, children, and adults. It is designed for use as a "hands-on" instructional program for new staff as well as a refresher course for experienced health professionals.
