Dear IAC Express reader,
Would you please help the Immunization Action Coalition (IAC) by completing a
5-minute survey?

As most of you know, for more than a decade, IAC has been a partner of the
Centers for Disease Control and Prevention (CDC). CDC currently provides IAC
with support for three major activities--publishing IAC Express and Ask the
Experts, as well as creating ready-to-print immunization education materials
for healthcare professionals and their patients.
Under the requirements of our CDC grant, we must evaluate the impact our
activities have on your immunization work. To do this, we have prepared an
online survey about IAC Express, Ask the Experts, and the following two
ready-to-print educational pieces:
